Sinusitis : Surgery is sometimes needed... See a rhinologist (an ENT sub-specialist) to sort out your options. In my practice around 15% of patients with your symptoms ultimately require surgery. Allergy , gerd , aspirin, sub-optimal medical treatment can all be to blame. If significant issues are not treated polyps can rapidly recur. I saw a patient who had undergone 63 surgeries for polyps - aspirin!

Medical tx: Middle ostia and eustachian tube obstruction may be tx medically. Localnasal or short term steroids are worth a trial.
