Chiari : No if you mean 3 mm. The cut off for chiari 1 malformation is 5mm. There are some that treat the so called "chiari 0" but this is controversial. I agree with dr oro comment. If 3 cm with or without a syrinx please see a neurosurgeon.

3 cm or 3 mm?: It depends. If it is 3 cm as you state, that is severe (30 mm) and you should be concerned. If instead you mean 3 mm, then it depends if you have a syrinx. If you do not have a syrinx, there is no concern.
