Yes: Unfortunately, most all forms of birth control have a failure rate -- even bilateral tubal ligation and vasectomy. For instance, there could be an "accessory vas deferens" on one side, giving your husband 3. When he had a "successful vasectomy" with 2 vas specimens confirmed, bam!, you're pregnant. Make sure his semenalysis shows no sperm present -- not even dead forms. Then you'll know for sure.
