Heparin is used: To achieve rapid full anti coagulation. Warfarin is then introduced, but takes 5-7 days to establish a therapeutic level.

Initially Heprin: We initially use Heparin as it anticoagulate immediatly while Coumadin (warfarin) takes at least 3-4 days before it works!
