It almost sounds: Like you have a neuroma. It is possible the scarring is independent of your numb 3rd toes. I would recommend you see a podiatrist for further work up.

Tough one: Maybe if you slept with a boot at night which keeps the feet straight instead go bending foreward with covers it could help. For pain you could consider tramadol in divided doses up to around 400 mg. It acts also to block ascending pain signals. If pain treated an snri like Cymbalta (duloxetine) would be helpful. Make sure you are sleeping so muscles relax, Clonazepam if necessary, f/u c doc, treat mood 2.
