Many causes possible: Ultimately a formal history and exam by an orthopedic surgeon is probably the best option for correct diagnosis. Many things can cause severe shoulder pain such as rotator cuff tears, bursitis/tendinitis, labral tears, shoulder instability, frozen shoulder, fractures, arthritis, contusions/direct trauma, cervical spine and nerve impingement in the shoulder as well.
