Multiple causes: Proceed from most likely causes, such as rotator cuff tear, AC joint impingement , Labral tear. Other possibilities include sub acromial bursitis, bicipital tendinitis and supra scapular neuralgia. Do not forget to ask if there is any pain radiating from the neck to the shoulder, in which case a C4-5 cervical disc herniation should be ruled out by MRI.
