Depends: Consult your primary doctor.Depends how much weight you lost, how much skin you have now.Skin elasticity changes w age.Give skin some time to readjust, and work out.Muscle growth (esp pectoral, deltoid, latissmus, &abs) can reduce saggy appearance.Even if you choose surgery, reducing some sag now can translate to smaller (= less traumatic) operation.Creams:mostly ineffective.(thanks to j. Packard).

Limited options: After bariatric surgery, patients often lose a significant amount of weight. With massive weight loss, the skin doesn't retract back enough, leaving lax, hanging skin. The best option is usually to remove the skin via a panniculectomy or an abdominoplasty. I have not been too impressed with noninvasive skin tightening technologies, but others may have more experience with them.

Tummy tuck is best: There are unfortunately no proven methods to tighten skin significantly after weight loss other than a tummy tuck (abdominoplasty), so be very wary of nonsurgical options marketed for that. Talk to a plastic surgeon, who will tell you what does and does not work there.

Extended tummy: After massive weight loss it is more likely that you will benefit from an extended tummy tuck or even a belt lipectomy in order to accomplish your goals. Removal of tummy skin only will leave you with unsatisfactory skin laxity in the hips and lwoer back.

No Good Ones: Unfortunately after extreme weight loss surgery, extended tummy tuck and lower body lift surgery are the best at correcting the skin laxity issue.
