Need to see Ortho: If common treatments have not helped, i would recommend seeing an orthpoedist so they can take proper x-rays to make sure there is no fracture.

May not be strain: Injuries to the proximal metatarsal and to the carpal metacarpal joint are often not simple sprains but, can be associated with fractures to the bones involved or significant disruption of the joint. Presistent pain and swelling should be evaluated the x-rays and , at time , more advanced imaging such as MRI or ct scan. I would recommend seeing an orthopaedic surgeon, possibly a hand specialist.
