Good question: Hard to answer this :o) high blood pressure predisposes you to heart disease, kidney damage, and stroke. Plaque build up - causes heart disease, kidney damage, and stroke. Also other blood vessel problems such as in the legs. I suppose, on average, would say that plaque build up (atherosclerosis) would be worse, although both need to be treated.
