Knee mri says wide separation of the lateral meniscal horns presumably due to radial tear. arthroscopy found no tear. still in pain, what next?

Repeat: Have an mr arthrogram and if + it was missed. It happens to the best of us. Also b sure the proper knee was imaged and scoped. The artrogram will show a tear better if it is - then if symptoms persist, one must keek looking 4 a cause.

Arthroscopy is gold: Generally speaking arthroscopy is gold standard assuming the doctor was complete and assesses both anterior and posterior root anchors. If it was normal, then pain is probably from another's source.....Perhaps arthritis, synovitis, etc.
