Cancer: Unfortunately , the cancer has advanced and affected other organs, including areas around neural tissue. Unclear what neural structures are involved. Your best guide is the surgeon and oncologist.

Difficult disease: Cholangiocarcinoma (cc) is a difficult disease to treat. Sometimes, small cc tumors can be surgically removed. In your case, the fact that the cancer has spread to the lymph nodes, and the fact that some of the tumor could not be removed would suggest that the survival will be limited after the operation. Discussing chemo and radiation is important in these cases.

Likely recurrence: Cholangiocarcinoma can spread by way of lymphatic channels to lymph nodes and along nerves that run in the wall of the bile duct. Both of these findings by the pathologist raise the likelihood of recurrence. Most worrisome, though, is when the entire detectable cancer cannot be removed. Leaving cancer on the hepatic artery assures recurrence. Chemotherapy +\- radiation may improve the outlook.
