Maybe a fissure: I can't be sure what she has without examining her, but i suspect she may have an anal fissure (a small cut which may be difficult to see due to skin folding in) which can be very painful. Fissure can happen if she recently had some hard stool and forced it out too quickly etc... Hemorrhoid cream and/or topical anesthetic dermablast may help. Give her some fibers. Consult doc for eval.
