Possible, not common: Diverticulitis is an infection of an outpouching (diverticulum), most commonly in the sigmoid colon (last part of the colon before the rectum). The sigmoid colon is in the left lower abdomen, so pain here is most common. Pain can also be in the pelvis, in the back, or in the right abdomen (with right colon diverticulitis, which is rare).

You can: If diverticulites perforated toward the back and formed an inflammatory mass pressing on the back it will cause back pain, though more commonly it causes pain in the left lower quadrant . Hope this helps.
