Time to seek help: If you haven't already done so an evaluation by a pediatric endocrinologist is in order. A 5 yo at this size is in danger of lifelong disability if he's not put on a better pathway soon.

Semi-emergency: This is morbid obesity and grandson is in severe danger of developing type 2 diabetes. He already will be unable to physically keep up with his peers. He needs to consult with a nutritionist. Then keep strictly to recommended calorie count. Can have no juices, limit beverages to water & skim milk. Insist on a glass of water before any meal. Lock up food & refridgerator. Snacks: only raw vegetables.
