Unlikely to work: The sperm count and morphology matters a lot as well. I assume he saw a urologist who examined him and determined he did not have a varicocele. Clomid (clomiphene) can work in women but it is more likely to improve sperm count. You may be looking at ivf with icsi as your only real option to conceive.

Serious abnormality: Sperm with only 3% motility is very poor. Try to get a reason if at all possible. Clomid (clomiphene) will likely not help in this situation but the drug is safe and inexpensive so a trial is not harmful. The sperm cycle is about 3 months so you must wait for results from med. Agree that if no motility improvement then ivf/icsi probably needed.
