Cultures: Routine blood cultures usually solve this matter, but it depends on the specific circumstance. It's very rare for the heart itself to get infected (with bacteria -- viruses are more common); it has a rich blood supply that defends it well. Usually when the heart is infected with bacteria it's usually on an artificial valve, a pacemaker, a damaged valve or some such.
