Hard to tell from...: Your question. Pelvic physical exam may be able to feel abnormal ovaries, but not diagnostic of cancer. Pelvic sono takes sound wave picture of ovaries and can give better picture (especially in overweight or obese women where ovaries can't usually be felt on physical exam). Sono may find suspicious ovaries, direct further testing, e.g. Biopsy.

Ovarian cancer: A pelvic exam can help a doctor find ovarian cancer. Imaging (ultrasound or ct scan) can also help the doctor. If a mass is found, a ca 125 can help a doctor triage you to a gyn oncologist if it is elevated (higher risk of ovarian cancer).
