I got a cast on my foot because of an achilles tendon injury and now i have severe pain on the top of my foot. why is this happening?

If the cast is a: "soft cast" rather than a hard one, walking on it will cause the soft cast to pressure the instep area, that area on the top of your foot where the leg meets the foot at the ankle, and can cause irritation. If you're in a soft cast, keep your walking to a minimum and yes, by all means, call your doctor. It might need to be changed and extra padding applied.
