Many possible ways: Renal arteries may arise from the aneurysm, they might be aneurysmal, there is usually some degree of narrowing of the origin of the renal arteries, they might be occluded, the left real veins might be behind the aortic aneurysm.

Aneurysm and kidney : Many abdominal aortic aneurysms start below your kidneys and therefore don't actually affect them.
