What does one do for gamekeepers thumb that has been untreated for 3 1/2 months?

Depends on how torn: I would recommend MRI or ultrasound of the thumb. If the joint is unstable, you definitely need surgery to prevent chronic instability and possibly arthritis. If it's a tiny partial tear, sometime it can be casted or splinted. See your orthopedist/sports medicine doc to further evaluate.
