Depends on symptoms: Usually anal fissures can be treated medically. The problem is they tend to recur. If your fissure is chronic and causing alot of symptoms or prevents you from doing things you like to do then surgery may be an option. Discuss this further with your surgeon.

May be : Most people have terrible pain and need surgery for this when medications fail. If u don't hurt too bad and u may not need surgery or may even have wrong diagnosis. Sometimes hemorrhoids can have similar presentation. Even for these, most people do not need surgery unless severe symptoms. Also, avoid scratching as it will keep irritating. Try soaking in warm water and drying the area 4 to 5/day.
