Musculo-skeletal: The differential can be varied but since you feel well otherwise i would suspect that this pain is being caused by a musculo-skeletal strain. The possibility of an inflammation of the pleura, the covering of the lung and what is called pleurisy. This problem can also cause pain on coughing and is usually associated with an infection.
