Depends: Cramping pain early in pregnancy is common and is thought to be a sign of the uterus growing or getting ready for implantation of an embryo. However, it is important to rule out an ectopic pregnancy (outside of the uterus). This causes severe pain and can be dangerous to the woman.

Maybe: At 4 weeks your period should just now be due. Are you certain that you are pregnant? Pain in the right lower quadrant in early pregnancy can be normal or can be something serious like appendix or a tubal pregnancy. You need to see a doctor immediately.
