Associated with PSC: Individuals with ulcerative colitis have an association with a cholestatic liver disease called primary sclerosing cholangitis (psc). Make sure you get your liver tests checked on a regular basis and see a liver specialist to discuss further.

Hepatitis B: Having a hbsag means you have hepatitis b. I would recommend you speak to a liver specialist to run some more tests to see if you need to be on oral antiviral treatment.

GI disease: Those with ulcerative colitis have an association with psc (primary sclerosing cholangitis). This can be determined by seeing a liver specialist. Liver disease in general doesn't lead directly to lower back pain, so i recommend you ask your pcp.
