Nothing: How are you determining the pulse and the heart rate? The usual problem is extra heartbeats which can be picked up by ecgs and machines like ecgs (exercise bikes with the metal patches, heart rate monitors, etc) but which generate a relatively weak pulse which is hard to feel and difficult for blood pressure cuffs and pulse oximeters to detect.
