A good interview : A mental health assessment involves an interview by a mental health expert to help identify your problem. It usually starts with open ended questions and then more specific questions are asked to narrow possible reasons for your problem. Sometimes questionnaires are used to identify problems like depression or adhd. Remember the assessment is to help you so don't be afraid to ask questions.

It varies: Some clinicians do testing as part of their evaluations, while others rely solely on conversation. Conversation is always the center point of the evaluation though. Every person has their strengths and difficulties; the clinician's job is to find out what yours are. People are often defensive and try to make themselves look good in evals, which almost always works against them, so be honest.
