At least 12 month: The optimal duration of treatment for mac is not known; however most expert would treat for 12 months after obtaining a negative sputum culture.

Need more details: Mycobacterium avium complex (mac) is associated with individuals who have severely decreased t-cell immunity. If you do not have HIV or aids, then you likely need to seek the help of someone to diagnose your underlying immune difficulty. Depending on your immune system, the treatment may vary.

See below: It is typically continued for 1 year after the first negative sputum sample. However, sometimes it is very difficult to clear and you may need to be on suppressive therapy for life.
