Anytime: Anytime anyone suffers a cardiopulmonary arrest acls is useful. This can be due to a loss of breathing or their heart stops. They may also have a heart rhythm that causes their pulse to be so low it does not provide effective circulation (hypotension); acls is useful in this setting as well.

For cardiac arrest: Acls is used to stabilize, and reverse, an underlying cardiac arrest. In other words, when a person loses consciousness due to a pathological heart rhythm problem, basic CPR will maintain the heart pumping blood to the brain and other vital organs, but acls is needed to correct the abnormal heart rhythm, and whatever is causing it.
