A family history is : The biggest risk factor followed by being middle aged and then being a smoker and out of shape -whlie being overweight does not help -weight is more of a risk in terms of back pain. Occupations that involve vibration load to the spine may also increase risk such as truck drivers.

Pelvic girdle injury: Between 10 & 30 percent people with chronic low back pain have pain generation from sacroiliac joint (sij) (medical literature research). Sciatica common in people with sij dysfunction (personal observation). To my mind sij most common source of chronic sciatica, & from pelvic presacral plexus impingement from sij dysfunction. Women with joint hypermobility syndrome (jhs) are prone to sciatica.

Overweight: Being overweight and also being in a job that require constant motion/twisting/bending at the waist, lifting heavy objects etc. Other times it can be as easy as tying your shoelaces or getting out of bed. Best bet is to eliminate or reduce the obvious issues like weight and overuse issues at work.

Integrative care: A herniated disc can cause back pain as well as foot pain by physically pressing on the nerves that travel to the foot and by irritating them through the extrusion of pro-inflammatory disc materials. Physical therapy and other conservative measures, including medication management, are considered first line treatment. If these fail, your physician may recommend epidural steroid injections.
