Difficult to answer: Could be something as simple as a muscle injury. Is his CF under control? Does he have signs or symptoms of infection? Also, GI problems are common in CF and it could even be referred pain from the GI tract. It is very difficult to say, and his physician can give you a better idea, knowing his history.

Chest pain/CF: The pain may be from a number of different causes, including muscular, cardiac, inflammation./infection. If the pain occurs when inhaling, the pain may be pleuritic and related to inflammation of the lining of the lung. I hope that this helps.
