Yes: As is the case with any test, if it is not done properly the results can be misleading. The technician doing the test has to have certain skills or the test can be misleading or just not of good enough quality to give the information being looked for. "garbage in, garbage out" they say.

Yes: Echocardiogram is a test administered and interpreted by human beings, so mistakes are certainly possible.
