Without : Without an exam it is difficult to determine the nature of the problem. There may be several reasons for the symptoms ranging from gum disease exposing the root of the tooth to an infection. The only way to tell for sure is to see you dentist for an exam.

Root canal: Over just one tooth like that, it may be an infected tooth -- abscess. Please see your dentist. You can't treat this by yourself.

Infection: Sounds like an infected tooth so go to dentist and find out. Generally gum disease is not painful.
