Depending : Depending on where the knots are. From what you are describing, sounds like tendonitis in the wrist. If it's in your wrist over the base of the thumb, it may be dequervain's tendonitis. This is a very common form of tendonitis in the wrist from repetitive lifting (boxes, babies, etc..). The initial treatment for this include use of a thumb brace and anti-inflammatories.

With : With no information about your age or sex, i'm going to have to make a guess. I'm going to guess that you are more than 40 years of age. All of us get arthritis at the base of the thumb, just like we all get grey hair. Women get it sooner than men, but we all get it. Your symptoms are classic for thumb base arthritis. A "knot" or fullness at the base of the thumb. Aching with gripping and turning. A hand surgeon can tell you for sure if your arthritis has arrived and give you some advice on managing it.
