Unfortunately : Unfortunately it seems you have severe arthritis in your shoulder. Sometimes a certain incident or event may precipitate or aggravate the condition, or you may have torn the remainder of your rotator cuff tendon. But clearly the arthritis and rotator cuff tear has been gradually worsening over many years. You could benefit from a consultation with an orthopedic surgeon for treatment including cortisone injections, physical therapy and possibly surgery.

Diagnosis: The spurring can occur in several areas including the ac joint and the shoulder joint. Spurs also take a long time to develop and I am concerned that we do not have a clear diagnosis or root cause for your shoulder pain.
