I : I do otoplasty as an out patient surgery - either at the surgical center or at the hospital, as per patients preference. There is a difference in price - hospital is a little more expensive.

Otoplasty : Otoplasty is done mostly as outpatient surgery or in an accredited office suegical suite. A young child might be done at the hospital, because they do better with general anesthesia. I do my botoplasties in my aaaasf accredited facility under local anesthesia only for adults or IV sedation if they request it. Very unusual to get an infection in a procedure like this or any cosmetic procedure if it is done by a board certified plastic surgeon or facial plastic surgeon.

It : It is unfortunate that your friend had an infection, but infections do occur with any type of surgery and believe it or not they are much more frequent in hospital patients than in a clinic setting.
