Incision : Incision placement for neck liposuction depends a lot on the surgeon's preference and where the fat is that needs to be removed. In my own practice i always place a small incision under the chin. I will sometimes add access incisions up behind the earlobes as well. Probably what's more important than incision placement is the ultimate results. Review your potential surgeon's before and after photos to make sure you like the results her or she gets.

As : As stated above the choice of incision placement depends upon the surgeons preference, areas that need to be addressed, and whether there is the presence of an existing scar. Hopefully you have been thoroughly evaluated by an appropriately trained and experienced surgeon. Things that they will consider, in addition to location of the fat deposits, is the condition of the overlying skin and underlying muscle groups. Many times liposuction alone will not result in an acceptable cosmetic result if there is redundant skin and / or dehisence (separation) of the underlying muscle fibers. These problems will need to be addressed with a surgical procedure where the muscle fibers are tightened and some skin is removed allowing for the creation of a nice tight smooth neck contour.
