Possible: If you had a previous episiotomy or previous tear it is certainly possible to have a laceration happen again but not a guarantee that it will. I find many patients on their second and third deliveries will have no tears at all. I do not cut routine episiotomies either.

It depends: It will depend upon the size of the new fetus and the experience the OB MD has in managing crowning at the time of delivery. The larger the infant and the less experience the OB MD has, the greater likelihood of an extensional tear of the episiotomy with the previous scar tissue and repair.
