No: If you do not want an episiotomy then have that conversation with your doctor well before your labor starts and write it into your birth plan. Ask questions about your doc's episiotomy rate and why he or she does them. Most docs do not routinely cut them anymore. If it is important to you then your doctor should respect your wishes and not cut one unless there is an urgent need.

It depends: It depends if not having an episiotomy is important to you or not. Some OB MD's are so concerned about perineal tears that they routinely do the procedure. Other OB MD's are trained to support and massage the perineum during crowning to avoid the procedure. Talk to your present OB MD about your concerns.
