Considering : Considering how long you had the infection together with your underlying medical condition, it would suggest that you go back to the dentist who extracted the tooth. It just may be post operative swelling, but it could also be an infection that is spreading. Please call at once.

I : I agree that this could be postoperative swelling, but with your history of an infection and the description of the location of your discomfort you should have your dentist refer you to a board certified oral and maxillofacial surgeon immediately to rule out spread of the infection.
