This : This is difficult to answer without a clinical examination so i would see a dermatologist. These could possibly be seborrheic keratoses, actinic keratoses, sun spots (lentigos), or other possibilities. Read about options for sunspots and the other conditions at the link below. A doctor with experience with skin problems should take a look and see what they think it is, and then can work on a solution. Obviously, losing weight and stopping smoking are important for more than cosmetic reasons, and you should take this opportunity to seriously consider things that will improve your health overall.
