I : I am answering this five days after you asked it. A child who has a temperature of 104 who is significantly lethargic (could be a mental status change) should be seen asap.

Yes: High fever and lethargy can be symptoms of a virus, or a serious bacteria infection. When she is treated for an infection, the antibiotic should be working within 3-4 days. If a fever occurs after that, or the symptoms worsen, there could be a new infection. See your doctor if you haven't already done so.
