Both : Both the nausea and fever can be from an infection, especially with a white count of 22, 000. You really need to talk with your brother's surgeon to find the cause, which sometimes does remain hidden. Your brother has had fairly extensive surgery and his surgeon is the best person to determine where this infection is located. Dr. D makes some good points about other possible sources of these symptoms, however.

It : It is difficukt to say. It does sound like he has an infection. But it also sounds like he has far advanced melanoma. His immune system may not be able to mount a good response. He may not have much time left. Ask his doctors for a family meeting. That should let you all get the answers, as limited as they may be, that you seek.
