Children : Children typically receive their first tetanus shot at 2 months, the second at 4 months, and the third at 6 months, the fourth between 15 and 18 months, and the fifth between 4 and 6 years of age. Then, the next dose is between ages 11 and 12 years old, and then up to every 10 years afterwards. I would clean the wound with soap and water, and then take your child to your regular doctor. Probably your child is up to date on his tetanus shots - but your child's doctor would know for sure (although he wouldn't have received the 5th dose of the primary series). Your doctor can evaluate the wound and the piece he fell on - in general, if the metal is not rusty or "dirty" (i.e. Was lying in a cow pasture) it should be okay. However, seeing your doctor is a good idea to see also if the scrapes and the wound require stitches, and possibly topical or oral antibiotics. Good luck! it's hard watching them learn to ride, isn't it? I was chewing my nails the whole time...

If : If your child is up to date on all the recommended vaccines, the he should be current on his tetanus immunity. It is the "t" in the dtap vaccine and he would have received a booster between 12 and 24 months of age. Whether you seek medical care or not depends on the status of the wound. If it is minor and without signs of infection, routine first aid care is all that is necessary.
