Several: Gout is caused by a problem of purine( a type of protein) metabolism. We all produce purine but it is the build up of it in the body that may cause a gouty attack. For certain people they may produce too much of it and or they may not get rid of it fast enough so it build up in the body usually around the joints( big toe). Poor kidney function, eating too much fish/meat and drinking are factors.

Joint urate deposits: Gout is an arthritis. The patients have elevated uric acid in the blood. The uric acid is metabolized to the sodium salt of uric acid which is called monosodium urate. Monosodium urate crystals are deposited throughout the body tissues and this is called total body urate stores. These urate crystals can be found in joints and stimulate the body protection cells to cause inflammation = gout!

Labs: Presumptive diagnosis of gout can be made by exam (swollen warm joint), labs (elevated serum uric acid levels) and history, definitive diagnosis requires analysis of an aspiration from the joint demonstrating appropriate "crystals".

Often not Rx fully: It's a very good question, firstly because it is often not rx correctly, and because often people that suffer from this often don't think they could ever be in a pain-free state. But that should be the goal. Hydration most important to prevent uric acid kidneys stones or acute gout. Allopurinol & uloric lowers serum uric acid levels; benemid (probenecid) inc excretion; Colchicine + nsaids rx acute attacks, etc.

Uric acid: When uric acid goes into the joint and precipitates, it forms gouty tophi which can eat away at bone causing punched out lesions.
