How does a doctor determine if the knee pain is from the joint or a nerve?

Good question: There are many ways to try to make the distinction. Nerve pain is often asscoiated with a certain distribution, tingling, numbness, or other associated processes such as weakness, muscle atrophy or back disease. There are specialized ways to test nerves. Joint pain is often characteristic and the pain can be provoked with certain tests. Also a jt injection with pain relief indicates the source.

Knee: ...By performing a thorough musculoskeletal exam and a neurological exam of the lower extremities the physiatrist (pm&r specialist) can determine the cause if the pain and recommend proper diagnostic testing and treatment.

Xray or MRI: Both may need to be done in knee and back. Generally though joint pain is specific location in pain around the knee but generally nowhere else. Sciatica/nerve pain can travel through the leg but generally starts from the spine. See a spine/sports specialist that can help you with this.
