Splint: Sometimes for moderate to severe pain i treat with a steroid injection. However the most important treatment is wearing a forearm splint daily for 3 months. Even when it feels better wear it for 3 months, or the pain often comes back. I use the band-it splint; can get on line. Voltaren (diclofenac) 1% gel and occupational therapy can also be helpful.

Time, activity mod: The condition tends to become chronic and becomes difficult to eradicate, no matter what the treatment. You've probably had standard treatment, including pt and injections. Specific soft tissue mobilization techniques have been helpful, and "blood patch" or platelet rich plasma injections have been coming in to vogue. Surgery is reserved for the most chronic & severe cases.
