Noncardiac causes: Some non-cardiac causes of chest pain include reflux from the esophagus (gastrointestinal esophageal reflux disease - gerd), costochondritis, and pleuritis. Shoulder pain could be from your neck from a pinched nerve or from the shoulder itself. A detailed history and physical exam with some additional tests by a meticulous doctor could sort it out.
