Never?: Unusual question to answer. There is nothing to be gained from screening asymptomatic people for blood clots. On the other hand, if you've had a blood clot, it is occasionally ok to look for resolution of your clot when determining how long to treat. Even then its not clear that this has any benefit.

Only if symptoms: The only reason to have a screening ultrasound of your veins to check for blood clots is if you have new symptoms in an extremity that make you concerned. Examples of reasons to get ultrasound screening is if you develop onset of extremity pain, extremity swelling, discoloration of an extremity, or if an extremity gets suddenly cold. Other reason is if you have a known condition that needs f/u.
