May require removal: If the large kidneys become more symptomatic (pain, pressure, bleeding) may require excision eventually, especially if they are at minimal function and you are already on dialysis (esrd).

ADPKD: These kidneys shrink only a small amount. Pain diminishes slowly. If transplantation is to be done, then one can be removed.
